Pharmacies. A narrow margin needs sharp figures.

For independent pharmacies and pharmacists with their own BV, from 5 to 25 people. Purchasing, stock, VAT at 9% and 21%, staff under the cao and your salary from the BV: Alvelion handles the accounting around it.

What matters in an independent pharmacy?

A pharmacy is care and trade at the same time. You supply prescription medicines, you advise patients and you sell over-the-counter products. Purchasing is by far your largest cost, and you agree your rates with health insurers.

At the Chamber of Commerce you are registered under SBI code 47.73, retail sale of pharmaceutical goods. Statistics Netherlands counted 1,360 businesses with this code on 1 October 2026. Six in ten are a BV or another legal entity.

According to the Dutch Healthcare Authority, 84% of pharmacies belonged to a chain or franchise in 2021. If you are independent, you make your own agreements and carry your own risk. That is where we help.

Where things go wrong for pharmacies

Do you recognise one of these situations? Then your accounting needs attention.

  • More stock, less margin

According to Statistics Netherlands, 66.6% of pharmacies' revenue went on purchasing in 2024. A year earlier it was 63.7%. The result fell from 5.7% to 5.2%. If your stock grows while the margin falls, your money is sitting on the shelf.

  • VAT at 9% and 21% mixed up

Medicines with a marketing authorisation fall under 9%. Over-the-counter products without a marketing authorisation, personal care and cosmetics fall under 21%. If that is not set up correctly in your till and your product file, your VAT return is wrong.

  • Supplier discounts without a written agreement

The Medicines Act prohibits inducements. Discounts and bonuses when buying medicines are allowed, under conditions. Without a written agreement you cannot show later what you received and why.

With your accounting in order, you see every month your margin, your stock and what the health insurers still owe you.

Your pharmacy, your choices

Independent or part of a franchise

Who makes the agreements?

As an independent pharmacy you negotiate with the health insurers yourself. The rates are free and the Dutch Healthcare Authority does not adjust them for inflation. In a chain or franchise many agreements are already fixed. We set out in figures what each route costs and brings in.

Own premises or rent

Where do your premises belong?

If you buy the premises, you choose whether they are held privately, in the pharmacy BV or in a separate BV. That decides the financing, the depreciation and what happens when you sell the pharmacy later. If you rent, your money stays free for stock and fittings.

We work out both choices before you sign.

How Alvelion handles the figures of your pharmacy

  1. Getting to know you and setting up

We look at your purchasing, your till, your contracts with health insurers and your legal form. We set up your accounting accordingly, with the right VAT codes for 9% and 21%.

  2. Keeping up to date

Purchases, sales, claims and bank are processed continuously. Every month you see your margin, your stock and what is still outstanding.

    Every month

  3. Payroll and tax returns

Your team's payslips under the Pharmacies cao, your own salary, the VAT return and the corporate income tax go out on time. So does the annual report in DigiMV.

  4. Looking ahead

A second pharmacy, an acquisition, a refurbishment or your pension? We work out in advance what it costs and brings in.

You work with one dedicated contact who knows your business, not a changing team or a general helpdesk.

Frequently asked questions

Medicines with a marketing authorisation under the Medicines Act fall under 9%. So do small-scale pharmacy preparations. Over-the-counter products without a marketing authorisation, personal care products and cosmetics fall under 21%. Unlike most care providers, you therefore simply charge VAT. That also means you may deduct the VAT on your purchases and investments.

The Medicines Act prohibits inducements, with exceptions. One of them covers discounts and bonuses when buying medicines. Conditions apply. Put every agreement in writing and have it checked before you sign. In your accounts you then see per supplier what you received.

You may only dispense medicines once the managing pharmacist is in the Health and Youth Care Inspectorate's register of established pharmacists. Have the stock valued separately from the goodwill. For tax, you write off goodwill at no more than 10% a year, so over at least ten years. If you buy through a new BV, it gets a new Chamber of Commerce number. Under the Healthcare Providers (Admission) Act you are then a new care provider.

Since 1 January 2025 the licence requirement has applied to almost all care providers that deliver care under the basic health insurance. Sole practitioners in a sole proprietorship are exempt. Existing providers received a licence by operation of law, but must meet the requirements. If your Chamber of Commerce number changes, for example with a new BV, you are seen as a new care provider and may need a new licence.

The Pharmacies cao applies to your assistants and other staff. It runs from 1 July 2024 to 31 December 2027 and has been declared generally binding. The pension runs through PMA, and you contribute to the Pharmacies Sector Fund. Pharmacists themselves are not covered by this cao. If you work as a pharmacist in a community pharmacy, you must take part in the SPOA pension fund. That also applies if you are employed or a DGA.

Yes. Pharmaceutical care is on the list of types of care for which a profit motive is allowed. The government is working on new rules for profit distributions in healthcare, with strict conditions and a maximum. That law does not yet apply. We keep track of it for you.

In principle, yes. Care providers submit an annual report on the previous year by 31 May. If you are small two years in a row, a lighter regime applies. That is the case if you meet two of these three: a balance sheet total up to €450,000, turnover up to €900,000 and fewer than 10 FTE on the payroll. Note: a pharmacy's turnover is often well above that limit.

Rates in community pharmacy are free. They are set in agreements with health insurers. Almost all pharmacies have a contract. The Dutch Healthcare Authority does not adjust free rates for inflation. So what you agree with the insurer decides most of your turnover. We show you what each contract brings in.

Taking over a pharmacy: what do you pay for stock and goodwill, and what must be in place before day one?

Anyone taking over a pharmacy usually pays for three things: the fixtures, the stock of medicines and the goodwill. You count the stock on the transfer date and value it separately. For tax purposes, goodwill is written off over at least ten years. And before you dispense the first prescription, the pharmacist in charge must be on the inspectorate's register.
