What does healthcare and wellbeing cover?

Everyone who looks after people's health or wellbeing, and in our case the vet too. We have worked out ten trades, from home care to pharmacy and from childcare to coaching.

We follow the classification of Statistics Netherlands (SBI 2025). Healthcare is under 86, residential care under 87 and non-residential social work under 88. Childcare has code 88910. Vets have their own code, 75000.

According to Statistics Netherlands, on 1 October 2026 there were almost 234,000 businesses in healthcare and social work, more than 42,000 more than five years earlier. 87% have one working person. Growing into a team means dealing with a licence, pay and an annual accountability report. That is where we help.

Where things go wrong in healthcare

Do you recognise one of these situations? Then your accounting needs attention.

  • One more person, but no licence

Since 1 January 2025 you need an admission licence if you provide care under the Zvw or the Wlz and you take someone on or hire a care professional. Even if you are a sole proprietor. The application costs €725 and in principle takes eight weeks.

  • Care money and private money mixed up

A care provider must keep care financially separate from other activities and be able to trace every payment. In September 2026 a home care BV was fined €100,000 for this, and the director personally €30,000. Payments to the director had not been recorded.

  • VAT-exempt without knowing why

Not all care is exempt from VAT. A temping agency always charges 21%. A self-employed doctor's assistant usually does too. Training without a therapeutic purpose is taxed. And if you are exempt, you cannot reclaim the VAT on costs and investments.

With your accounting in order, you know per funder what you earn, what is still outstanding and what you need to account for.

Solo practitioner or care provider

Solo practitioner

Do you provide all the care yourself?

You register with the Care Providers Portal before you start. You do not need a licence as long as you work alone as a sole proprietor. Often no annual accountability report either, as long as you provide all the care yourself.

Care provider

Does anyone work for you?

One employee or one hired care professional makes you an institution. A locum counts too. Then an annual accountability report by 31 May and separate flows of money come with it, and a licence for Zvw or Wlz care.

We set up your accounting so that the step from solo practitioner to care provider brings no surprises.

How Alvelion handles the figures of your practice

  1. Getting to know you and setting up

We look at who pays for your care and what is exempt from VAT and what is not. We set up your accounting accordingly, with care money kept apart from the rest.

  2. Keeping up to date

Claims, invoices, purchases and bank are processed continuously. You see per funder what you have claimed and what is still outstanding.

    Every month

  3. Payroll and tax returns

Payslips under your cao, the pension return, your tax returns and the annual accountability report in DigiMV go out on time.

  4. Looking ahead

A first employee, a practice BV or taking over a practice? We work out in advance what it costs and what it asks of you.

You work with one dedicated contact who knows your business, not a changing team or a general helpdesk.

Frequently asked questions

Often yes, but not always. Medical care with a therapeutic purpose is exempt if you have a BIG profession within your field, or training at an equivalent level. Care for clients with a Wlz assessment, a Wmo decision or youth care is also exempt. Taxed services include supplying temporary care staff, medical examinations, training without a therapeutic purpose and the work of a self-employed doctor's assistant without their own exemption. For exempt care you cannot reclaim VAT on your costs.

Every care provider registers with the Care Providers Portal before starting. Since 2025 you need an admission licence if you provide Zvw or Wlz care and you employ someone or outsource care. If you work alone as a sole proprietor, or only provide Wmo care or youth care, you do not. Medical specialist care always requires a licence, even in a BV of one specialist. Note: if you move from sole proprietorship to BV, you get a new Chamber of Commerce number and you are a new care provider.

Almost all care providers file an annual accountability report through DigiMV by 31 May each year. A self-employed professional who provides all the care personally does not have to. If you used a locum even once or had someone on the payroll, you do. A small provider can use the lighter regime for micro care providers: no management report and no auditor's report. Youth care gets its own regulation from 1 January 2027.

Yes, if the freelancer is genuinely self-employed. The court looks at all the circumstances together. In healthcare, the fact that a freelancer does the same work as your employees is not in itself proof of employment. Whether someone may send a substitute, the rate, the freedom to turn down shifts and genuine entrepreneurship weigh heavily. Note: a hired care professional makes you an institution, with a licence and an annual accountability report.

For many care businesses with staff PFZW is compulsory, for example in home care, disability care, youth care, childcare and GP care. Making a profit makes no difference. Some money from the government or from health insurance is enough. Some trades have their own fund: SPF for physiotherapists, SPH for GPs, SPMS for medical specialists and SPOA for pharmacists. If in doubt, for example with a sole proprietorship with staff, have it checked by PFZW.

For many practices you can, such as physiotherapy, GP care, dental care, psychological care and the pharmacy. For other care there is a ban on profit-making, for example medical specialist care in a private clinic. The government wants a new act to allow profit distribution in insured care and youth care under strict conditions and with a maximum. That act is not yet in force.

Usually not. The exemption is for care institutions whose profit stays in care. If the shareholders are the care professionals themselves, or their holding company, the BV does not meet that requirement. The Tax Administration said so in 2025 about a cooperative of GPs. A practice BV therefore pays corporate income tax as normal.

A municipality is a public body and must pay your invoice within 30 days. With health insurers and care offices, the term is in your contract. Always expect a gap: you provide care first, claim afterwards and then wait for the money. A claim without a valid assessment, decision or referral is not paid. You do pay wages and rent every month.

Pensions in the veterinary practice: SPD for the vet, PFZW for the assistant

In a veterinary practice, two pension schemes often run side by side. Every vet working in the Netherlands takes part compulsorily in the veterinarians' pension fund, with a premium of 26.2% in 2026. Assistants are not covered by PFZW's mandatory participation, but they are covered by PFZW if your practice is a member of the employers' association that signed the collective agreement. Who pays which part of the premium is set out in that agreement.

Working as a locum medical specialist under the NVZ and FMS model agreement: what do you need to do in practice?

The NVZ and FMS model agreement for locum work in the MSB was assessed by the tax authority on 1 March 2023. Anyone working under it does not have to pay payroll tax. The conditions are strict: cover for a named absent specialist, a maximum of six months and one six-month extension, medical work only, free to work elsewhere. Existing model agreements remain usable until 31 December 2029.

Mental health cao and PFZW: why does sole proprietorship or company make a difference for your staff?

A psychology practice with staff does not automatically fall under the mental health cao and the PFZW pension fund. The cao defines the employer as a legal entity, such as a private limited company or foundation, and has not been generally binding since 1 August 2026. PFZW excludes employers that only provide general basic mental health care without a medical specialist. Anyone growing or moving to a company must review both again.

Dental practice without a cao, but PFZW for the assistants: what do you arrange as an employer?

There is no cao for dental assistants and dental hygienists. The KNMT withdrew its employment conditions scheme at the end of 2022, and in 2026 there is still no oral care cao. So you agree the employment conditions yourself, within the law. The pension is settled, however: a dentist must register their dental assistants and senior dental assistants with PFZW. The contribution in 2026 is 25.90% of the salary above the franchise of € 17,283. You may register other employees voluntarily, but then everyone.

Regular locum or own practice: what is still possible as a self-employed GP?

The Tax Administration has been enforcing against false self-employment again since 2025. In 2026 it can impose penalties for intent or gross negligence and recover tax back to 1 January 2025. For GPs there is an approved model agreement for longer-term locum work, but it itself recommends a different agreement for regular locum work. A court ruled in 2023 that a locum at an out-of-hours service was not an employee. The LHV sees regular locum work disappearing and more GPs choosing employment or a partnership.
